High Country Fly Fishers Mentoring Sign Up

Registration and signed waiver is required to participate in High Country Fly Fisher Activities.

I understand and acknowledge that High Country Fly Fishers is not an insurer of my behavior, actions or participation in the event and that the organization assumes no liability whatsoever for personal injuries or property damages to me or to third persons arising out of my participation in the event activities. I hereby agree to release, waive, covenant not to sue, indemnify and hold harmless the organization, and all of their officers, employees, and agents from all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury, including death, that may be sustained by me or loss/damage to any property belonging to me arising out of participation in the above-named event. Knowing these risks, I freely and voluntarily participate in the event/activity and hereby agree to assume and accept any and all risks. I have adequate health insurance to cover the costs of treatment in the event of any injury. I shall pay any attorney fees/costs incurred by High Country Fly Fishers in enforcing this Agreement. If any portion of this Agreement is held to be invalid by a court of law, then it is agreed and intended that all the remainder shall, notwithstanding, continue in full force and effect.

I understand and acknowledge that I am granting High Country Fly Fishers permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of High Country Fly Fishers and will not be returned. I hereby irrevocably authorize High Country Fly Fishers to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge the High Country Fly Fishers from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

By providing my name, I agree to signing this agreement for myself and all 3rd parties I bring to the activity.
